Yesterday and today, the following form was distributed to all in-person students. We need these Consent to Test forms back ASAP, if you have not already done so.
In the 16th section of Executive Order 14 from Governor Lewis, it states:
(c) the school provides surveillance testing in coordination with and upon recommendations from the Tribal Health Department and COVID-19 Task Force
The first Surveillance Testing will be this Friday, March 18. All students must have a negative test in order to continue in-person.
This form will allow the testing entity to provide information to both Gila River Health AND Gila Crossing Community School.
If this form is not on file, then information can't be readily shared with us, and we may not get results necessary for students to continue in-person until these results are obtained.
