Welcome!

Thank you for choosing Gila Crossing Community School! To register a new student at our school, please;

1.     Download and complete the New Student Enrollment Form, Home Language Survey, 506 Form, Culture Letter and Medical Consent. You are also welcome to pick up a packet from the front office. Please see the following letter regarding enrolling a student.

Enrollment Letter to Parent/Guardian

2.     Bring the Enrollment Form along with the following documents to our school:

• Birth Certificate

• Immunization Record

• CIB (Tribal Enrollment)

• IEP Form (if required)

• Guardianship Documents (if applicable)

• For a transferring student, please bring the following:

  • Copy of the student's transcript with most recent report card, attendance records and behavior

    records

If your student is returning for the 24-25 SY,

please complete the Re-Enrollment Form and Medical Consent
